First of all I could tell by the mfd and voltage that it was for a fan motor, second there are no mfg. that use a start cap for a fan motor. run capacitors can come in many different shapes and sizes and can be either metal or plastic it doesn't matter as long as the mfd is +/- 10% and the voltage rating is the same or higher. In your case you need a 4mfd cap at 370 or 440 volts, either one will work perfectly. Do not use a 5mfd cap it is 25% higher than a 4 and will load up the start winding in the motor too much causing the motor to run hotter than normal.
